STEPS FOR DEPENDENT STUDENT VERIFICATION
Section A: Include all members in the household.
- You (the student)
- Parent(s) and step-parent if remarried
- Any siblings (if in college, please list name of college)
- Any other dependents that your parents will support at least 51% of their livelihood from July 2020-June 2021.

Section B: Student will indicate their tax return or income information for the 2018 tax filing year.
- Box 1- Student used the data retrieval tool while completing the FAFSA and no additional tax documents are requested on the 'My Dashboard' tab.
- Box 2- Student did file taxes in 2018 and will provide either a signed federal 1040 tax return (and any schedules if applicable) or IRS Tax Return Transcript. For an example of the 2018 tax documents click here. You can obtain a IRS Tax Return transcript by creating an account at www.IRS.gov and requesting the document electronically or by mail. You can also call the IRS at 1-800-908-9946 to request a transcript and it will be mailed to the address on file with the IRS.
- Box 3- Student did not work in 2018 and are not required to file taxes. No additional tax information is needed.
- Box 4- Student worked in 2018, but were not required to file a federal tax return. You will need to list the amount you earned and provide all 2018 W2's or 1099 from each place of employment.

Section C: Parent(s) will indicate their tax return or income information for the 2018 tax year.
- Box 1- Parent(s) used the data retrieval tool while completing the FAFSA and no additional tax documents are requested on the 'My Dashboard' tab.

- Box 4- Parent(s) worked in 2018, but were not required to file a federal tax return. You will need to list the amount you earned and provide all 2018 W2's or 1099 from each place of employment. Parent(s) will also need to request and submit a "Verification of Non-filing letter". For information on how to request the letter click here.

Section D: Student and Parent will need to provide an ink signature and date. *We cannot accept electronic signature.
